We explain the program in plain language, answer your questions, and get your consent. Nothing is signed until you say yes.
Meet your care coordinator
You'll get a name, a direct number, and a care plan built with your doctor during your first conversation.
Stay supported every month
Regular calls, medication reviews, and coordination with your providers. You can pause or leave the program at any time.

"I used to leave appointments with a list of questions I forgot to ask. Now I just call Denise. She already knows my medications and my history."
Am I eligible? What does it cost?
If you have Medicare Part B and two or more chronic conditions expected to last at least a year, you likely qualify for chronic care management. Medicare covers the large majority of the cost, and most patients with a supplement or Medicare Advantage plan pay nothing out of pocket.

Common questions
Does this replace my doctor?+
No. We work alongside your existing doctor and specialists. Everything we do is shared back with your care team so nothing falls through the cracks.
What does it cost?+
Chronic care management is a covered Medicare Part B service. Medicare pays roughly 80%, and the small remaining coinsurance is often covered in full by a supplement or Medicare Advantage plan. We review your exact cost before you enroll.
Which conditions count?+
Any two or more chronic conditions expected to last at least a year, including diabetes, hypertension, heart failure, COPD, asthma, kidney disease, arthritis, depression, and dementia.
How much of my time does it take?+
Usually one call of about 20 minutes each month, at a time that works for you. You can always call us in between.
Can I leave the program?+
Yes, at any time, for any reason, with a single phone call. There is no contract and no penalty.
